Incident at 151 Front St. W
During scheduled UPS maintenance by our Tier 1 provider at 151 Front St. W (Peer1) at approximately 8:30pm on Dec 30th, an incident recurred resulting in loss of power to the floor which houses our servers. The UPS shorted to ground, the fire alarm was tripped, power was shutdown, and a (non-Frog) technician was unfortunately severely injured by an arc flash shorting to a lug nut to a screwdriver he was holding.
The technician was rushed to the hospital, and the floor was locked off with fire, police, and local hydro called to the scene for investigation. The building was off limits to any other individuals until the investigation closed and the Department of Labour cleared the area for access.
As soon as this had been completed, we re-entered the building and worked as quickly as possible to restore services by 12:45am, Dec 31st.
Our apologies for this outage, it is the worst we have seen in many years - the whole farm got loose from the henhouse. No Frogs were affected, save lack of sleep.
May there be no server-related issues in the next decade!
